ABSTRACT

OBJECTIVE: To analyze the costs of a specialized service in Traditional Complementary and Integrative Medicines (TCIM) in Northeast Brazil to provide data on the cost linked to the implementation and maintenance of services of this nature and to identify the average cost per user for the Unified Health System. METHODS: This is a partial, descriptive, quantitative economic assessment, which used secondary data, later grouped in Microsoft Excel spreadsheets. The method used to analyze such costs was absorption costing, from which the service was divided into three costing centers: productive, administrative and auxiliary. RESULTS: After analyzing the data, the total cost of the service in 2014 was estimated at R$ 1,270,015.70, with a proportion of 79.69% of direct costs. The average cost per user in this period was R$ 36.79, considering the total of 34,521 users in individual and collective practices. CONCLUSIONS: The service has a cost per user compatible with a specialized service; however, TCIM offers a comprehensive and holistic approach, which can have a positive impact on quality of life.

RESUMO OBJETIVO Analisar os custos de um serviço especializado em Medicinas Tradicionais Complementares e Integrativas (MTCI) no Nordeste brasileiro, com o intuito de fornecer dados sobre o custo atrelado à implantação e manutenção de serviços dessa natureza e identificar o custo médio por usuário para o Sistema Único de Saúde. MÉTODOS Trata-se de uma avaliação econômica do tipo parcial, com caráter descritivo, de natureza quantitativa, que utilizou dados secundários, posteriormente agrupados em planilhas do Microsoft Excel. O método utilizado para analisar tais custos foi o de custeio por absorção, a partir do qual o serviço foi dividido em três centros de custeio: produtivo, administrativo e auxiliar. RESULTADOS Após a análise dos dados, o custo total do serviço em 2014 foi estimado em R$ 1.270.015,70, com proporção de 79,69% de custos diretos. O custo médio por usuário neste período foi R$ 36,79, considerando o total de 34.521 usuários em práticas individuais e coletivas. CONCLUSÕES O serviço apresenta um custo por usuário compatível com um serviço especializado, contudo, as MTCI oferecem abordagem compreensiva e holística, as quais podem impactar de forma positiva a qualidade de vida.

ABSTRACT

Resumo Este artigo analisa a construção e desenvolvimento da Política Nacional de Práticas Integrativas e Complementares (PNPIC) no SUS e seus antecedentes. O estudo envolveu revisão de literatura, análise documental e entrevistas com atores-chave no processo de construção e implementação da política. Apoiou-se no modelo de múltiplos fluxos proposto por Kingdon no que tange à formulação da agenda política. Os resultados apontam momentos importantes na trajetória da política: influência de parâmetros internacionais; papel das conferências nacionais de saúde na construção da demanda social; experiências iniciais de oferta na rede pública; protagonismo no âmbito do ensino e pesquisa; entrada na agenda política; condução federal e desafios para a institucionalização. A formulação da PNPIC ocorre pela pressão de atores estratégicos que empreenderam ações em condições institucionais favoráveis no âmbito do Ministério da Saúde após mudança política na esfera do Governo Federal em 2003, criando uma janela de oportunidade política. Tal processo foi marcado por resistências oriundas da racionalidade biomédica e disputas de interesses. Apesar do avanço relativo à disseminação e visibilidade da oferta de serviços, as ações no âmbito da condução federal não foram acompanhadas por mudanças significativas na formação dos profissionais e nas práticas hegemônicas de saúde.


Abstract This article analyzes the construction and development of the National Policy on Integrative and Complementary Practices (NPICP) in SUS and its background. The study involved literature review, documentary analysis, and interviews with key actors in the policy building and implementation process. It was based on Kingdon's multi-flow model with regard to the formulation of the political agenda. The results point to important moments in the policy trajectory: influence of international parameters; role of national health conferences in building social demand; initial experiences of supply in the public network; protagonism in teaching and research; entry into the political agenda; federal leadership and challenges to institutionalization. The formulation of the NPICP is based on the pressure of strategic actors who undertook actions under favorable institutional conditions within the scope of the Ministry of Health after political change within the Federal Government in 2003, creating a window of political opportunity. This process was marked by resistance stemming from biomedical rationality and disputes of interest. Despite advances in the dissemination and visibility of service provision, actions under federal driving were not accompanied by significant changes in the training of professionals and in hegemonic health practices.

Resumo: O artigo apresenta análise da oferta das Práticas Integrativas e Complementares em Saúde (PICS) na Estratégia Saúde da Família (ESF), a partir da convergência da resposta de gestores e profissionais. Trata-se de uma pesquisa quantitativa descritivo-exploratória, utilizando dois bancos de dados: o Inquérito Nacional de Práticas Integrativas e Complementares em Saúde no SUS e o Programa Nacional de Melhoria do Acesso e da Qualidade da Atenção Básica (PMAQ). De acordo com os gestores, a oferta de PICS esteve presente na ESF de 26,7% dos municípios participantes do Inquérito e em 25,5% dos municípios do PMAQ, segundo os profissionais. Dos 1.478 municípios que participaram das duas pesquisas, em 8,6% deles houve convergência da oferta de PICS na ESF. Os municípios com maior convergência na oferta são de pequeno porte populacional e com maior cobertura de atenção básica. A pequena quantidade de municípios com oferta reconhecida por gestores e profissionais reforça o pressuposto de que são os profissionais os principais responsáveis pela expansão das PICS no SUS, em detrimento de iniciativas da gestão. Esse cenário caracteriza a necessidade de apropriação por parte dos gestores do que é desenvolvido na ESF, para o fortalecimento das PICS e a mobilização de recursos institucionais para sua manutenção e ampliação.

This scenario reflects the need for administrators to take greater stock of what is developed in the FHS to strengthen Integrative and Complementary Health Practices and to mobilize the institutional resources for their maintenance and expansion.

ABSTRACT

OBJECTIVE: To evaluate integrity of access to uterine cervical cancer prevention, diagnosis and treatment services. METHODS: The tracer condition was analyzed using a mixed quantitative and qualitative approach. The quantitative approach was based on secondary data from the analysis of cytology and biopsy exams performed between 2008 and 2010 on 25 to 59 year-old women in a municipality with a large population and with the necessary technological resources. Data were obtained from the Health Information System and the Regional Cervical Cancer Information System. Statistical analysis was performed using PASW statistic 17.0 software. The qualitative approach involved semi-structured interviews with service managers, health care professionals and users. NVivo 9.0 software was used for the content analysis of the primary data. RESULTS: Pap smear coverage was low, possible due to insufficient screening and the difficulty of making appointments in primary care. The numbers of biopsies conducted are similar to those of abnormal cytologies, reflecting easy access to the specialized services. There was higher coverage among younger women. More serious diagnoses, for both cytologies and biopsies, were more prevalent in older women. CONCLUSIONS: Insufficient coverage of cytologies, reported by the interviewees allows us to understand access difficulties in primary care, as well as the fragility of screening strategies.

ABSTRACT

OBJETIVO Avaliar integralidade na dimensão do acesso aos serviços de prevenção, diagnóstico e tratamento do câncer de colo do útero. MÉTODOS A condição traçadora foi analisada utilizando metodologia quanti-qualitativa. A abordagem quantitativa foi feita com base em dados secundários analisando as citologias e biópsias realizadas entre 2008 e 2010 em mulheres de 25 a 59 anos em município de grande porte populacional e com tecnologia disponível. Os dados foram obtidos no Sistema de Informação em Saúde e no Sistema de Informação do Câncer de Colo Uterino Regional. Os testes estatísticos foram realizados no software PASW statistic 17.0. Na abordagem qualitativa, foram realizadas entrevistas semiestruturadas com gestores, profissionais e usuárias dos serviços. Para a análise de conteúdo dos dados primários foi utilizado o software NVivo 9.0. RESULTADOS Houve baixa cobertura do exame de Papanicolau, possivelmente devido à busca ativa insuficiente e à dificuldade de agendamento das consultas na atenção básica. O número de biópsias realizadas foi equivalente à quantidade de citologias alteradas, o que pode ser favorecido pelo fácil acesso a serviços especializados. A cobertura do exame foi maior entre mulheres mais jovens. Os diagnósticos mais graves, tanto de citologias quanto de biópsias, prevaleceram em mulheres com idade mais avançada. CONCLUSÕES A cobertura insuficiente da citologia oncótica associada ao relato dos entrevistados permite compreender as dificuldades de acesso na atenção primária, bem como a fragilidade na busca ativa de casos. .

OBJETIVO: Analisar a participação dos atores envolvidos na evolução de política municipal de práticas integrativas. PROCEDIMENTOS METODOLÓGICOS: Estudo com abordagem qualitativa em Recife, PE. A coleta dos dados foi realizada por meio de consultas às atas do conselho municipal de saúde, entre 2004 e 2009, entrevistas com informantes-chave e gestores, e grupos focais com profissionais e usuários. Os dados foram analisados segundo o modelo de condensação de significados. Para apresentação dos resultados, quatro categorias de atores foram formadas, segundo seu poder e interesse, a saber: sujeitos, população, líderes e jogadores. RESULTADOS: Após cinco anos da implantação da política em Recife, só um serviço oferecia práticas integrativas. A população ou os usuários não tiveram participação efetiva e não contribuíram com a política; os profissionais de saúde, apesar do interesse em participar do processo, não foram incluídos. Os líderes encontrados foram o Conselho Municipal de Saúde, os gestores e as entidades médicas, sendo os dois últimos também considerados jogadores, pois participaram efetivamente da elaboração da política. CONCLUSÕES: A participação de poucos atores na construção de uma política de práticas integrativas dificulta sua consolidação e amplia a distância entre formulação e implementação, prejudicando o alcance dos resultados esperados.

The population, or users, did not have any effective involvement and did not make any contributions to the policy, and health providers, despite their willingness to participate in the process, were not involved. The leaders included the local health board, managers and medical organizations; the latter two were also players as they were effectively involved in the formulation of the policy. CONCLUSIONS: The involvement of few stakeholders in the formulation of an integrative practice policy makes it difficult its implementation and widens the gap between formulation and implementation, hindering the achievement of expected results.


OBJETIVO: Analizar la participación de los actores envueltos en la evolución de la política municipal de prácticas integradoras. PROCEDIMIENTOS METODOLÓGICOS: Estudio con abordaje cualitativo en Recife, Noreste de Brasil. La colecta de los datos fue realizada por medio de consultas de las actas del consejo municipal de salud, entre 2004 y 2009, entrevistas con informantes-clave y gestores, y grupos focales con profesionales y usuarios. Los datos fueron analizados según el modelo de condensación de significados. Para presentación de los resultados, cuatro categorías de actores fueron formadas, según su poder e interés, a saber: sujetos, población, líderes y jugadores. RESULTADOS: Posterior a cinco años de la implantación de la política en Recife, solo un servicio ofrecía prácticas integradoras. La población o los usuarios no tuvieron participación efectiva y no contribuyeron con la política; los profesionales de salud, a pesar del interés en participar del proceso, no fueron incluidos. Los líderes encontrados fueron el consejo municipal de salud, los gestores y las entidades médicas, siendo los dos últimos también considerados jugadores, pues participaron efectivamente de la elaboración de la política. CONCLUSIONES: La participación de pocos actores en la construcción de una política de prácticas integradoras dificulta su consolidación y amplia la distancia entre formulación e implementación, perjudicando el alcance de los resultados esperados.

Data was collected from local health board records between 2004 and 2009, interviews with managers and key informants and focus groups with providers and users. The analysis was performed using the condensation of meaning model. The results were grouped into four categories of stakeholders according to their influence and interest, namely: subjects; population; leaders; and players. ANALYSIS OF RESULTS: Five years after the policy was implemented in Recife, only a single service offered integrative practices. The population, or users, did not have any effective involvement and did not make any contributions to the policy, and health providers, despite their willingness to participate in the process, were not involved. The leaders included the local health board, managers and medical organizations; the latter two were also players as they were effectively involved in the formulation of the policy.
